Important Information for Patients and Guardians:

We encourage all patients and their legal guardians to read this document thoroughly. Understanding our policies helps us provide the best care for you and your family.

 Tardiness, Cancellations & No-Show Policy:

 We put in a lot of effort to schedule your appointments. We understand that things come up, and we’re here to work with you. To ensure timely service for all our patients, please note the following requirements:

  • Confirm Your Appointment: Please confirm your appointment at least 48 hours in advance; otherwise, it may be canceled.
  • Cancellation Notice: We need at least 48 business hours’ notice for cancellations via call, text, or email to accommodate other patients.
  • Missed Appointments: If you miss an appointment without proper notice, you may incur a $100 fee.
  • Credit Card Information: If you violate the above policies, you will need to provide credit card information to book future appointments.
  • Missed Appointments Policy: Missing three appointments may lead to permanent dismissal from our practice.
  • Deposits for Repeated Cancellations: Patients with frequent cancellations will be required to pay a deposit for future appointments.
  • Late Arrivals: If you arrive more than 10 minutes late, you may not be seen.


Insurance Claim Submission Policy & Assignment of Benefits

Your insurance plan is a contract between you, your employer, and the insurance company. We are an in-network provider, allowing you to enjoy discounts and benefits. While we can provide a discount amount, we do not have the information to tell you your exact out-of-pocket costs; we can only give you an estimate. Your final financial responsibility will be determined by your insurance company after we submit a claim.

Please be aware that your insurance may not cover all services, and plans can have various stipulations that affect coverage. These may include exclusions, waiting periods, and non-covered procedures. Insurance companies can also change their policies without notice.

Our team will try to gather relevant information from your insurance, but it’s your responsibility to know and provide the most accurate and current details about your dental plan.

Pre-authorizations are not guarantees of payment, though they can provide useful information. You should follow up with your insurance to ensure timely processing of your claims or pre-authorizations.

If your insurance has not paid a claim within six months of the service date, the expected amount will become your responsibility. If your insurance does not cover any service, that cost will also be your responsibility.

Outstanding Balances: Any balance remaining after 12 months will be reported to credit bureaus and sent to collections.

Ridgeview Dental Care will submit claims and necessary documents, such as Explanation of Benefits and X-rays, to your insurance plans, which will make payments directly to us.
