Discounts on procedures apply only if the patient is a candidate for that specific procedure and the procedure falls within the scope of our general dental practice. Please note that some procedures, such as, but not limited to, extractions, implants, and root canals, may be referred to specialists, and the discounts offered by this plan will not apply to those costs. Eligibility for discounts is determined by our dental providers and applies only if the patient is a candidate for the procedure and it is within the scope of our practice. Each adult in the household will need to pay the $99 annual fee to access the benefits. **Children under 18**, as legal dependents, can enjoy the plan’s benefits at no additional cost, provided at least one adult in the household is an active member. By enrolling, you agree that: 1. **Each adult** (18 or older) must pay the $99 annual fee. 2. **Children under 18** may benefit at no extra charge if a parent or guardian is enrolled.
